Hello lovely ladies. This is a consultation form that you need to fill out before your appointment, if you haven’t booked in yet please fill out this form and then click here to get yourself booked in!

Have you had lash extensions before?

Do you wear contact lenses?

Do you have frequent eye irritation, itching or watery eyes?

Have you had surgery around your eyes in the last 6 months?

Do you consent to photos of your lashes/face being put on my social media?

Are you undergoing chemo therapy or radiation?

Immune system is weakened, hair in poor condition and client could react to adhesive vapours

Are you currently taking anti-histamines on a regular basis?

(ie. you will likely have one on the morning of your appointment) Anti-histamines can mask reactions causing you to have an allergic reaction to the adhesive when your tablet wears off.

Have you had any of the following in the last 2 weeks?

Chemical peels, micro-needling, plasma pen, laser hair removal on your face, vampire facials, micro dermabrasion. The treatments listed all require healing time and can cause you to have a reaction to the adhesive. I also rest my hands on your head during the treatment which would interfere with healing of the forehead so I would not recommend lashes within 2 weeks of getting forehead botox.

The salon address is 55 Highfield Road, Corfe Mullen BH21 3PB. When you arrive you will likely spot the Barbershop first, please use the parking directly infront of the Barbershop - you will find the salon to the right of the Barbers. Please come straight in!

We have toilet facilities in the salon that you can use before you lie down as lashing usually takes about 1.5-2 hours. Please arrive on time. If you are more than 10 minutes late I will not be able to complete a full set of lashes but you will still be charged the same. If you are on any medication or treatment that affects your hair growth please note that this may reduce the retention of the lashes if they are falling out. If you are taking medication or are pregnant/breastfeeding please be aware that this may increase or change your body’s way of reacting with the adhesive. Your sensitivity to lash glue can also change if you have had Covid-19.

If you would like a patch test this consists of applying a few individual lashes on the corner of each eye. An allergic reaction would occur on BOTH eyes - a reaction on a single eye is due to an error in application or failure to perform correct aftercare on the lash.
